Rehabilitation of burn scars and scars caused by other forms of trauma using pressure therapy with elastic compression garments is widely acknowledged to be effective, non-invasive, and safe. With the proper measurements and appropriate features and options, your patient can benefit from compression therapy. • Hair protectors cover the ear openings
• Holes can be cut in the garment to allow for ponytails and pigtails
• Ears may be exposed or covered. The ear holes may look like slots, but when stretched the areas become circular to fit the ear.
• Although hook & loop fasteners are more frequently used, zippers can be used instead. The cross-seams on the scalp give a roundness to the contours of the garment.
• Lining material can be inserted at the chin or crown.
• Material cut in a V-shape can serve as a nose bridge.
• Hook & loop tabs can be used to attach the face garments to a vest.
• Collars may be attached to chin straps and face masks to provide extra comfort for sensitive areas.

Important!
Custom-made pressure garments provide that constant, even pressure necessary to aid in preventing hypertrophic scarring and disfigurement. To be effective, the garments should be worn 24 hours a day, every day, until the scar tissue is mature, usually 9-14 months post-burn. A pressure garment maintains effective pressure for 2-3 months and then must be replaced. Alternating between several garments will aid in lengthening the life of the garments, although, in some cases of weight change, or spurts of rapid growth in children, more frequent re-measurement and replacement may be necessary.

• Expansion panels made of insert material can be used instead of zippers for glove closures.
• Pockets for inserting additional pressure material can be sewn on either the palmar or dorsal side, using lining material.
• Web spacers are fitted on the underside and in between the fingers of the glove. Softer insert material can be used across the metacarpophalangeal joints to reduce abrasion.
• The glove can be made a part of the sleeve.
• Gloves for toddlers may be made of close-knit material on the palmar surface, and insert material dorsally.
• Garments can be made to cover just the thumb and index finger. Inserts can be placed in the thumb web space especially if that area is unburned. Darts can be sewn in the area of the thenar eminence to pull the thumb into opposition. A similar procedure can be used for little fingers, but sometimes just twisting the material can accomplish the same purpose.

Note: Shoulder flaps are often needed to hold up the glove to axilla, gauntlet to axilla, and sleeve to axilla garments.
• Gussets in the axillary area help prevent abrasion of this sensitive area.
• Gussets are preferred over linings.
• If one arm is unburned, a short sleeve over the upper arm is preferable to a sleeveless garment that tends to rub or dig in.
Sleeves can also have built-in pads. If the elbow needs additional protection or pressure, a built-in pad can be useful. For women, a strap or loop can be attached to a bra if only one sleeve is needed.

• Gussets in the axillae help prevent abrasion to this sensitive area (gussets are better than linings at abrasion control).
• Insert material can extend the entire length of a sleeve from armpit to wrist, or from wrist to waist in one continuous piece.
• Pads can be built in to sleeve elbows or other sensitive areas.
• Zippers are optional for lower extremity garments.
• Vest and suit necklines can be standard, scooped, V-neck, or turtleneck. A scooped neckline on an adult garment extends 3 inches below the sternal notch unless a larger neckline is requested. The V-neck can be as deep as 5 - 6 inches. Turtlenecks extend up from the sternal notch either 1, 1½, 2, 2½, or 3 inches and may have a separate hook & loop closure.
• If the burn is limited to the abdomen and waist, a tank top combined with a brief can be designed. This combination would allow the patient to wear normal clothes.
• If donning the garment over the foot is particularly difficult for the patient, a heel insert or a full-length insert panel along the side of one or both legs may be used instead of a zipper.
• The metatarsal heads is the best place to end a full-length leotard.
• Leotards and briefs can be designed with an open crotch.

• Pockets for inserting additional pressure material can be sewn on dorsal side, using lining material.
• Where donning the garment over the foot is particularly difficult, a heel insert or a full-length insert panel along the side of one/both legs may he used instead of a zipper. The additional stretch of the material may allow the patient to pull on the garment without zippers.
• Foot mittens with no seams on the soles are recommended. They may be made of lining material to minimize friction and shearing.
